Bit of a gues as I don't have bar end mounted mirrors so I'm guessing its mounted like a normal bar end weight.
Is there an Allen bolt in the end of the weight? Loosen that a slightly, push the weight back in and tighten it up again.
The ones Ive seen had an expanding section of either rubber that expanded when it was shortened by the tightening of the bolt or a split metal collet shaped section that a tapered plug pulled into and expanded it.

I've no idea how it would just slide out without the mirror also moving round from the wind when it was loose though.

When putting the indicator back in- I can’t tighten, it just spins and spins. Indicator still works

There may be a rubber insert in the bar with a threaded metal centre. The bolt connected to the bar end may cause this to pinch and thus grip the inside of the bar.

The bolt probably holds the bar end in place, which then holds the mirror in place.

This is a guess based on limited visibility!

Sounds about right - you need it to be done up a bit so that the rubber grips the inside of the bar sufficiently well to allow you to tigthen it. It's the gripping which is allowing you to turn.

Can you pull the indicator out of the bar? Can't really see how it all goes together....

There's your problem, you should have, and need to get the thing out and find out why it's gone wrong before jamming it back down the bar.
I'm going to guess it's a smidge tight so getting it out now might be tricky, but somewhere inside your bar the nut or expanding section that the indicator screw into have had a paddy.

They will be they've no way of escaping, they should be on the other side of the collar he tapped back into the bar.
Best guess is the nut has gone walk about if he can't get it to engage the threaded bar on the indicator.
